Nurse Practitioner

We are seeking a highly skilled and compassionate Registered Nurse to join our Pain Management team at Advocate Christ Medical Center. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and contributing to the development of our clinical programs.
Responsibilities- Utilize the nursing process to assess patient needs, develop individualized care plans, and evaluate the effectiveness of interventions.
- Collaborate with patients, families, and healthcare teams to provide holistic care and promote patient-centered outcomes.
- Anticipate and respond to changes in patient status, mobilizing resources and coordinating care as needed.
- Implement strategies to reduce patient risk and increase safety, incorporating evidence-based practices and regulatory standards.
- Actively coordinate discharge planning, applying an interdisciplinary approach and promoting continuity of care.
- Provide ongoing evaluation of patient, team, and unit outcomes, driving quality improvement initiatives and promoting a culture of safety.
- Demonstrate effective communication, feedback, and conflict resolution skills, promoting collaboration and teamwork among healthcare professionals.
- Assume a leadership role in the care team, delegating tasks and promoting a culture of accountability and respect.
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) certification issued by the American Heart Association.
- Completion of an accredited or approved program in Nursing.
- Typically requires 1 year of experience in clinical nursing.
