VIA Health Partners is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality end-of-life care to patients and their families. As an LPN, you will play a vital role in delivering patient-centered care, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities
- Conduct thorough nursing assessments to identify patient needs and develop individualized care plans.
- Provide direct patient care, including administering medications, wound care, and other interventions as needed.
- Collaborate with the healthcare team to coordinate care and ensure seamless transitions.
- Document patient progress and communicate effectively with families and healthcare providers.
- Participate in ongoing education and training to stay current on best practices and evidence-based care.
Requirements
- Current, active LPN licensure in the state of North Carolina.
- Minimum 1 year of experience in hospice or palliative care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Computer proficiency and ability to navigate electronic medical records.
