Job Description

We are looking for a compassionate and experienced Social Worker who can provide psychosocial support to patients undergoing dialysis treatment. Your primary responsibilities will include:

  • Conducting comprehensive assessments of patients' psychosocial status and developing individualized care plans to address their needs
  • Collaborating with the interdisciplinary team to promote positive adjustment, rehabilitation, and improved quality of life for patients
  • Providing education and counseling to patients and their families on topics such as end-stage renal disease, transplantation, and insurance benefits
  • Maintaining accurate records of patient interactions and progress

Requirements

To be considered for this position, you must possess:

  • A Master's degree in Social Work from an accredited institution
  • A valid state license to practice social work
  • At least 0-2 years of experience in a related field

  • Ability to work effectively in a fast-paced environment
