The Nurse Coordinator for Oncology Multispecialty Care Centers plays a vital role in navigating patients and families across care settings according to established patient care plans. This position ensures that patients/families have the necessary information, understand their treatment options, and are able to adhere to the medical plan.
Key Responsibilities:- Patient Assessments: Conduct comprehensive assessments to identify current and projected needs, facilitating effective communication among patients, families, and healthcare providers.
- Clinical Practice and Care Coordination: Develop education plans based on assessment findings to address teaching/learning topics, styles, and special needs, promoting clinical research initiatives by identifying candidates for open trials and collaborating with Research Nurses.
- Bachelor's Degree in Nursing or higher preferred; Connecticut RN license required.
- Oncology experience is highly preferred; Oncology Nursing Society certification (OCN, AOCN) desirable.
