Fresenius Medical Care North America is seeking a highly skilled Master Social Worker to join our team in providing exceptional care to patients with chronic kidney disease.

The estimated salary for this role is around $65,000 - $80,000 per year, depending on location and experience.

About the Role

This entry-level MSW position requires working closely with healthcare teams to promote positive adjustment, rehabilitation, and improved quality of life for our patients. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

The Purpose and Scope of this role includes:

  • Providing psychosocial services to patients treated by Fresenius Medical Care, including in-center and home dialysis patients.
  • Collaborating with healthcare teams to inform, educate, and support staff in understanding the emotional, psychological, and behavioral impact of chronic kidney disease on patients and families.

Key Responsibilities

1. Patient Assessment / Care Planning / Counseling
  • Assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes.
  • Participate in care planning with patients and healthcare teams to identify effective interventions for improving patient outcomes.
  • Monitor patients' progress and provide supportive counseling services as needed.
2. Quality
  • Provide psychosocial support and interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level.
  • Participate in monthly Quality review meetings with interdisciplinary teams to report on quality indicators related to adherence.
3. Patient Education
  • Assess patients' knowledge of kidney disease and work with patients, families, and healthcare teams to provide education tailored to individual learning styles and needs.
  • Facilitate transplant referral processes and collaborate with interdisciplinary teams on transplant waitlist management.
4. Patient Admission and Continuity of Care
  • Review patient rights and responsibilities, grievance procedures, and other facility policies with patients and/or their representatives.
  • Support clinics in scheduling to maximize adherence and adjustment.

Requirements

  • Masters in Social Work degree from an accredited institution.
  • State-required license to practice social work.
  • 0-2 years of related experience.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.
