Join Our Team as a Heart Failure Cardiologist

About the Role

We are seeking a highly qualified Cardiologist with expertise in Heart Failure Medicine to join our Regional Cardiovascular Medicine team. The successful candidate will be responsible for creating and implementing systems of care for high-quality and efficient hospital care of heart failure patients, as well as population health management of outpatients with heart failure.

Key Responsibilities
  • Develop and implement systems of care for heart failure patients
  • Manage population health of outpatients with heart failure
  • Identify and follow up with patients needing advanced procedural treatments
  • Collaborate with multidisciplinary teams, including primary care, cardiovascular specialists, and procedural teams
  • Utilize electronic medical record population health analytics to inform care decisions
Requirements

The ideal candidate will be a Board Certified/Board Eligible Cardiologist with advanced subspecialty training in Heart Failure Medicine. A faculty appointment at a rank commensurate with experience is available at the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University.
