As a Home Health Registered Nurse (RN) with LHC Group, you will have the opportunity to provide compassionate and personalized care to patients in their own homes. Our team of experienced nurses works closely with patients, families, and other healthcare professionals to develop and implement individualized care plans that address the unique needs and goals of each patient.

**Responsibilities:**
   Conduct initial and ongoing assessments of patients' physical, emotional, and social needs;
   Develop and implement individualized care plans that address the unique needs and goals of each patient;
   Coordinate with physicians, agency personnel, patients, families, and community resources to achieve continuity of patient care;
   Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes;
   Maintain accurate and timely documentation of patient progress and care activities;
   Participate in ongoing education and training to stay current on best practices and industry developments;
   Promote a culture of safety, quality, and compassion throughout the organization.

**Requirements:**
   Current RN licensure in the state of practice;
   Current CPR certification;
   Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation;
   Strong communication and interpersonal skills;
   Ability to work independently and collaboratively as part of a team;
   Basic computer skills and proficiency in electronic medical records;

**Salary:**
   Competitive salary range of $85,000 to $90,000 per year, based on full-time employment and maximum productivity;
