Estimated Salary Range: $40,000 - $60,000 per year, depending on experience and qualifications.

Responsibilities:

* Provide exceptional care to patients in their homes, promoting independence and dignity.
* Collaborate with healthcare professionals to develop and implement personalized care plans.
* Communicate effectively with patients, families, and caregivers to ensure seamless care delivery.
* Participate in ongoing training and education to stay up-to-date on best practices and industry developments.

Requirements:

* High school diploma or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.
* 1-2 years of experience in a healthcare setting, preferably in home care or a related field.
* Valid driver's license and reliable transportation.
* Ability to lift 25 pounds and perform physical tasks as needed.
