We are seeking a highly skilled and compassionate Nurse Practitioner to join our Assertive Community Treatment (ACT) team at A Caring Alternative, LLC. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive care to individuals with severe and persistent mental illness.
Key Responsibilities- Service Coordination: Provide case management services for assigned clients, coordinating and monitoring the activities of the individual treatment team.
- Individual Supportive Therapy: Offer individual supportive therapy and symptom management, ensuring timely adjustments to treatment plans as needed.
- Crisis Intervention: Provide on-call crisis intervention services, covering nighttime hours and serving as a backup to evening and weekend staff.
- Medication Management: Organize and manage the system of getting medication to clients, integrating medication administration tightly into individual treatment plans.
- Collaboration with Psychiatrist: Coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment.
- Education and Experience: LPN, RN, or Nurse Practitioner licensure, with a minimum of one year of experience working with adults with serious mental illness.
- Computer Skills: Ability to perform tasks on the computer, including working with Microsoft 360, Excel, and Word.
- Valid Driver's License: Possess a valid driver's license, auto insurance coverage, and reliable transportation.
- Criminal Background Check: Pass a criminal background check and other regulatory checks.
- Competitive Payrate: Receive a competitive rate of pay.
