Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna. As a key member of our healthcare team, you will provide high-quality patient care and support to our clients in their homes.

Job Summary:

The Licensed Practical Nurse (LPN/LVN) is responsible for delivering and coordinating quality patient care in compliance with physician orders. This includes observing and assessing patient condition and care needs, documenting all activities and assessments, and participating in the development and implementation of the nursing care plan.

Responsibilities:
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs.
  • Document all activities, assessments, and nursing actions.
  • Participate in the development and implementation of the nursing care plan.
  • Provide education to patients and families regarding disease processes, self-care techniques, and prevention strategies.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Working Conditions:

The Licensed Practical Nurse (LPN/LVN) will work in a variety of environments, including patient homes, and may be exposed to allergens and other conditions. The position requires the ability to lift 50 pounds, sit, climb stairs, and have visual and hearing acuity.
