We are seeking a dedicated and compassionate Case Manager II to join our Homebase Homelessness Prevention Programs team. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of individuals and families at risk of homelessness.

Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized plans to address clients' needs.
  • Provide case management services, including housing subsidy assistance, tenant/landlord mediation, and advocacy/benefit entitlements.
  • Maintain accurate and up-to-date case records, including progress notes and client contact information.

Requirements:

  • Bachelor's degree in Human Services or related field required.
  • One year of experience in social services or direct service provision preferred.
  • Bilingual skills in Spanish a plus.
