Homecare LPN Position

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care to a pediatric patient in their own home in Monroe Center, IL.

The patient is a 5-year-old female who requires G-tube/trach and vent care at night. Our team is committed to providing exceptional care and support to ensure the patient's comfort and well-being.

• Utilize the nursing process to plan, implement, and evaluate patient care and services provided to the patient.
• Initiate the plan of care when directed and request revisions as the patient's status and needs change.
• Administer medications and treatments as prescribed and monitor their effectiveness.
• Review the plan of care and identify areas for healthcare teaching, care coordination, goal setting, and discharge planning.

• Current Licensed Practical Nurse (LPN) License for the state in which the nurse practices.
• Current Health Certificate.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior LPN experience preferred.
