We are seeking a skilled Case Manager Utilization RN to join our team at Recruitics JB. As a key member of our multidisciplinary team, you will work collaboratively with physicians to coordinate and screen for the appropriateness of admissions and continued stays.
Key Responsibilities- Develop and implement individualized plans of care to ensure continuity, quality, and appropriate resource use.
- Collaborate with physicians, other healthcare professionals, and patients/families to coordinate care and ensure a smooth transition from hospital to home.
- Conduct daily clinical reviews to identify opportunities for improvement and optimize resource utilization.
- Act as a liaison between in-patient facilities and referral agencies to facilitate seamless care transitions.
- Provide education and support to patients and families to promote self-management and independence.
- Participate in quality improvement initiatives to enhance patient outcomes and reduce costs.
- Two (2) years of combined RN experience in an acute care setting or case management required.
- Completion of an accredited RN training program and current California RN license.
- Basic Life Support certification.
- Bachelor's degree in nursing or a related field.
