Position Details:
During a visit that may last up to one hour, Matrix providers assess and evaluate a member's current health status, medical history, medication compliance, social circumstances, and other risk factors. This comprehensive evaluation provides invaluable insights into a member's overall health that are often missed during standard office visits. The Matrix Comprehensive Health Assessment is designed to enhance care quality and allows us to address multiple care gaps in a single appointment.

Key Responsibilities:
What to Expect as a Matrix Provider:
- Conduct comprehensive assessments for adults and geriatrics, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and accurate documentation in accordance with Matrix protocols.
- Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and relevant ancillary medical personnel.
- Engage with Primary Care Physicians (PCPs) on patient education and ensure appropriate follow-up.
- Provide services across various settings, including home visits, skilled nursing facility visits, and telehealth consultations.
Position Type: PRN/Part Time - Flexible Schedule
Compensation: $95.00 per completed assessment
Work Environment: Home, skilled nursing facilities, telehealth, and other community settings.
Work Hours: Flexible scheduling options available, including days, evenings, nights, and weekends.
Benefits: PRN/Part Time providers are eligible for 401(k) and limited voluntary benefits.
Qualifications:
Essential Requirements:
- Master's Degree or equivalent experience with successful completion of NP licensure.
- Master's Degree or equivalent experience with successful completion of PA licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain it.
- Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- A minimum of one year of experience as a Nurse Practitioner or Physician Assistant, with the ability to travel as needed.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- A positive, outgoing personality, and the ability to work as part of a team while adapting to frequent changes.
Travel Requirements:
- Ability to travel, possess a valid state driver's license, and maintain adequate automobile insurance coverage.
- Travel is necessary to meet patients in their homes.
