Nurse Practitioner

CVS Health Careers is seeking a highly skilled Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide evidenced-based care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.
Key Responsibilities:- Provide patient-centered care, including accurate assessment, diagnosis, treatment, and management of health problems.
- Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centric, longitudinal care.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in clinical care team huddles and contribute to the development of care plans.
- Document patient care within an electronic health record, adhering to current policies and procedures.
- Work autonomously, prioritizing and solving problems, taking initiative, and advocating for patients and the practice.
- Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
- Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing.
- Current National Board Certification in Family Practice.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Self-motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practice.
- Effective verbal, written, and electronic communication skills.
- Outstanding organizational skills and ability to multi-task.
- Initiative, problem-solving ability, adaptability, and flexibility.
- Ability to work remotely in a clinical care team culture.
- Ability to work without direct supervision and practice autonomously.
- Is proficient with information management and technology.
- Willingness to obtain multi-state licensures.
- Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
- While performing the duties of the job, the Primary Care NP is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses.
- Specific vision abilities include the ability to view and read a computer screen and other electronic devices.
