A highly respected Home Care agency is seeking a Registered Nurse (RN) with exceptional administrative and leadership skills to contribute to their team in the Ventura area.
Job Summary:This RN will be responsible for visiting patients in their homes, assessing patient needs, and updating care plans.
Key Responsibilities:- Develop and maintain patient care plans in accordance with agency policies and procedures.
- Conduct patient visits to ensure that care plans are being followed and updated as necessary.
- Communicate with patients, their families, and other healthcare professionals to coordinate care and ensure patient satisfaction.
- Conduct regular reviews of patient charts to ensure that all required documentation is accurate and up-to-date.
- Collaborate with other healthcare professionals to ensure that patients receive the highest level of care.
- Work with agency leadership to develop and implement quality improvement initiatives.
- Ensure that all patient information is kept confidential and in compliance with HIPAA regulations.
- Participate in agency meetings and trainings as required.
- Must be a Registered Nurse with an active California nursing license.
- Strong organizational and administrative skills with the ability to prioritize tasks and meet deadlines.
- Excellent communication skills with the ability to build rapport with patients and their families.
- Ability to travel throughout the Ventura and surrounding area as required.
- Strong understanding of HIPAA regulations and the ability to maintain patient confidentiality at all times.
