Location: Coos Bay, OR
Work Schedule: Full Time, 40 hours per week, Monday to Friday or Tuesday to Saturday, with occasional on-call duties
Compensation: Starting at $45 per hour, with potential increases based on experience, along with a sign-on bonus of up to $7,000 and relocation assistance available.
Key Responsibilities:
- Deliver comprehensive nursing care for a designated group of terminally ill patients, ensuring coordination among various internal care team members and collaboration with external healthcare providers.
- Evaluate documentation for all newly assigned patients and liaise with admissions personnel or other care team members to maintain continuity of care.
- Adapt to diverse and unpredictable environments, including personal residences, nursing homes, assisted living facilities, and adult foster homes.
- Facilitate and coordinate multidisciplinary care in an interdisciplinary manner, engaging with other professional team members to provide holistic service addressing physical, emotional, social, and spiritual needs.
- Current RN License in Oregon is mandatory.
- CPR and BLS certifications are required.
- Experience in hospice care is essential.
- Ability to meet physical demands, including lifting up to 50 pounds.
- Willingness and capability to drive in various road and weather conditions.
