The estimated annual salary for this position is $95,000 to $135,000, depending on experience.

Job Description

We are seeking an experienced Advanced Care Provider to join our team as a Primary-Care Provider to cover Clinton, Madison Heights, and Clawson facilities. The successful candidate will be responsible for providing high-quality patient care, working in coordination with an LTC Collaborating Physician, and contributing to the overall quality of care and patient outcomes.

Requirements and Qualifications
  • A valid and unrestricted PA medical license in Michigan is required.
  • A valid DEA license in the state of Michigan is also required.
  • Candidates must have completed a valid U.S. certification program.
Job Structure
  • This is an in-person position, requiring providers to commute directly between home/facilities.
  • Nurse Practitioners work in coordination with an LTC Collaborating Physician.
  • Nurse Practitioner is responsible for the day-to-day care of their patients.
  • After-Hours on-call is shared among all LTC Providers.

  • $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance.
