CAN Community Health, a leading provider of HIV services, is seeking a dedicated and compassionate Licensed Practical Nurse Level 1 or Medical Assistant Level 1 to join our team in Cape Coral, FL.

About the Role:

This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality patient care and contributing to the mission of CAN Community Health. As a Licensed Practical Nurse Level 1 or Medical Assistant Level 1, you will work under the direct supervision of a Medical Provider and Registered Nurse, providing basic nursing care to patients and enhancing their quality of life.

Responsibilities:
  • Promote and practice CAN Community Health's mission, vision, and values, and adhere to its policies and procedures.
  • Ensure confidentiality is maintained regarding patient/client information in accordance with HIPAA, professional, and departmental standards.
  • Perform patient triage, including assessment of vital signs, measurement of anthropometric measures, history taking, and reviews patient chart to ensure all necessary documentation is present, current, and complete to facilitate provider visit and documents them accordingly in the EMR.
  • Process medication refills per medical provider's order and in compliance with CAN Medication refill and EMR Policies (LPNs only).
  • Perform venipuncture, obtain specimens, and process these according to laboratory specific requirements.
  • Perform care team intake procedures under the responsibility of the Registered Nurse (LPN only).
  • Perform intravenous catheterization and administer intravenous fluids and medications per medical provider order, upon completion of IV Infusion Certification (LPNs only).
  • Practice safety, environmental, and/or infection control methods according to established protocols and procedures.
  • Document all patient care in an accurate and timely manner, and ensure compliance with grant requirements (340B, HRSA, Ryan White).
  • Request diagnostic test results from external laboratories and/or other healthcare providers.
  • Provide assistance with referrals and coordinates services with other departments, healthcare facilities, organizations, and federal and local programs.
  • Maintain and promote confidentiality and HIPAA standards regarding patients and/or family in accordance with professional and departmental standards.
  • Maintain and stock inventory of clinical supplies, medications (including samples), and vaccines, and complete the necessary logs per CAN policies and procedures.
  • Delegate tasks to other support staff as needed and follow up to ensure these are completed effectively.
  • Adhere to ethical and legal standards of professional nursing practice (LPNs only).
  • Maintain nursing licensure/certification up-to-date and in compliance with state and regulatory requirements.
  • Continues professional training and education to advance knowledge and skills of HIV/AIDS, STIs, and other topics related to the care of CAN mission patients.
  • Participate in continuous quality assurance and safety compliance activities as delegated by nurse manager, practice administrator.
  • Support quality program processes and objectives; participate in local quality team as assigned.
  • Utilize resources and supplies in a cost-effective manner.
  • Demonstrate courtesy, compassion, professionalism, and respect in all interpersonal relationships with CAN patients, clients, staff, and visitors.
  • Participate and strive to achieve assigned Annual Performance Objectives as established by CAN leadership.
  • Continuously demonstrate clinical competency with nursing skills and safety procedures and utilize universal precautions where appropriate.
Requirements:
  • LPN/LVN graduate of an accredited school of nursing currently licensed to practice in the state where clinic is located.
  • Medical Assistant Certification issued by an accredited institution approved by the local state, or a minimum of 3 years' experience working in a healthcare setting.
  • Basic Cardiac Life Support (CPR) Certification.
