Key Responsibilities:
  • Identify and collect referral information to support patient eligibility for care
  • Collaborate with Area Vice Presidents of Sales to foster relationships with key account staff
  • Make calls and referrals to service key accounts and patients
  • Communicate with patients and families regarding services offered
  • Interact with hospital staff from various departments
  • Consolidate and review patients' records upon referral

  • Explain care options to patients and their families

Requirements:
  • High School Diploma/GED
  • 3+ years' practical experience in healthcare
  • LPN/RN License preferred

  • Experience in Case Management in a clinical setting preferred
  • Dependable vehicle, valid driver's license, and current auto insurance
