At CenterWell Senior Primary Care, we are seeking an exceptional Primary Care Physician to join our team-based care environment. As a valued member of our team, you will work as a lead, overseeing the care of our patients and collaborating with other healthcare professionals to deliver high-quality patient care.

Job Summary

The Primary Care Physician is responsible for evaluating and treating patients in accordance with standards of care. This includes following established levels of medical care and quality metrics, as well as monitoring patient outcomes through data analysis and chart reviews.

In addition to providing direct patient care, you will assist in coordinating patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care. You will also participate in daily huddles with the care team and collaborate with Regional Medical Director and Center Administrator to set a tone of cooperation and professionalism.

Responsibilities
  • Evaluate and treat patients in accordance with standards of care
  • Follow established levels of medical care and quality metrics
  • Monitor patient outcomes through data analysis and chart reviews
  • Assist in coordinating patient services, including specialty referrals and hospital coordination
  • Participate in daily huddles with the care team
  • Collaborate with Regional Medical Director and Center Administrator to set a tone of cooperation and professionalism
Requirements
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice
  • Eligible and willing to obtain licenses in other states in the region of assignment, as required
  • Graduate of an accredited MD or DO program
  • Excellent verbal and written communication skills
  • Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients
  • Fully engaged in the concept of integrated team-based care model
  • Willingness and ability to learn/adapt to practice in a value-based care setting
  • Superior patient/customer service
  • Basic computer skills, including email and EMR
Preferred Qualifications
  • Board Certification or Eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Active and unrestricted DEA license
  • Medicare Provider Number
  • Medicaid Provider Number
  • Minimum of two to five years directly applicable experience
  • Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
