Job Summary

We are seeking a dedicated and compassionate Community Health Worker to join our Enhanced Care Management program at Sierra Vista Child & Family Services. As a Community Health Worker, you will play a vital role in promoting community-based health education and prevention in underserved communities.

Key Responsibilities
  • Provide administrative support to lead care managers and engage eligible Enhanced Care Management members.
  • Coordinate services as part of a multidisciplinary team to promote preventive health services and prevent disease, disability, and other health conditions.
  • Develop and maintain relationships with community resources and organizations to provide access, advocacy, and linkage to clients and their families.
  • Utilize prevention strategies to promote stigma and discrimination reduction, suicide prevention, mental health awareness, and well-being.
  • Collaborate with a multi-disciplinary team and other community organizations to achieve program goals.
  • Maintain accurate records and tracking systems that meet agency, Joint Commission, and contract standards.
Requirements
  • High School diploma or GED required.
  • Proficient in Microsoft suite (Word, Excel, PowerPoint).
  • Strong computer skills and data entry abilities.
  • Knowledge of community resources and ability to work effectively with cultural/ethnic diversity.
  • Bilingual in Spanish preferred.
  • A valid California driver's license is required for staff with regular driving duties.
