Position Overview: The MDS (Minimum Data Set) Coordinator plays a crucial role in generating statistical reports mandated by relevant legal, accrediting, and licensing standards for healthcare establishments. This role involves compiling information and data from health records and personal evaluations to assess the quality of care and conduct utilization reviews.

Key Responsibilities:

  • Strategically plans, directs, and coordinates MDS evaluations and care plan services.
  • Conducts health assessments for all residents upon admission to formulate initial care plans and performs subsequent assessments quarterly.
  • Collaborates with healthcare providers and physicians to develop, implement, and assess individualized care plans for each resident.
  • Analyzes and accurately classifies diagnoses, procedures, and operations from health records using appropriate coding systems, ensuring precise data entry into the computer system.
  • Organizes and leads quarterly care plan conferences for each resident, involving family members and physicians.
  • Develops, implements, and oversees quality management, utilization review, and discharge planning by consulting with other healthcare professionals regarding resident care needs.
  • Reviews documentation quarterly to ensure compliance with established minimum standards for resident chart retention.
  • Adheres to all submission deadlines for reports and documentation as required by state and federal regulations.
  • Actively engages in the QAPI (Quality Assurance Process Improvement) program, fostering a proactive approach to quality care and services with involvement from residents, families, and staff.
  • Monitors compliance with accrediting and regulatory body criteria.
  • Participates in ongoing education to maintain credentials and enhance expertise in nursing and administration.
  • Demonstrates a high level of initiative and commitment to efficiently completing assessments with meticulous attention to detail.
  • Stays informed in the nursing field through participation in meetings, conferences, and workshops related to resident care and rehabilitation.
  • Willingness to perform RN duties as required.
  • Exhibits understanding and compassion for the elderly population.
  • Creates a positive atmosphere for residents in alignment with our core values.
  • Follows instructions and completes daily tasks as assigned, demonstrating initiative.

Specific Requirements:

  • Must possess a current, unencumbered nursing license in Iowa.
  • Must be proficient in reading, writing, speaking, and understanding English.
  • Ability to make independent decisions when necessary.
  • Capable of interacting effectively with personnel, residents, families, and the public.

Job Type: Full-time

Salary: $34.00 per hour
