Job Summary

We are seeking a skilled Physical Therapist to join our team at BayCare HomeCare. As a Physical Therapist, you will provide hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy.

Responsibilities
  • Evaluate and develop specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
  • Provide the appropriate level of clinical direction for Physical Therapy Assistants.
  • Plan, implement, manage and evaluate the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
  • Manage the activities of the multi-disciplinary team providing care to the patient.
  • Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
  • Develop, implement, and carry out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
  • Interpret agency policy to patients/families.
  • Review patients visits weekly and provide plan to PCS and team assistant, including anticipated weekend visits.
  • Inform team assistant of any schedule changes and communicate caseload and patient care issues to Patient Care Supervisor
Requirements
  • BLS (Basic Life Support)
  • PT (Physical Therapist) License
  • Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
Preferred Qualifications
  • Masters (Physical Therapy)
  • 1 year Home Care
