About the Role:

The Wellness Nurse is a key member of our senior living community, responsible for providing nursing triage, health education, and risk reduction strategies to maximize residents' health and quality of life. This role requires a compassionate and enthusiastic individual who is passionate about hospitality and senior engagement.

Responsibilities:
  • Assist residents in achieving their health and wellness goals, within the framework of the 8 Dimensions of Wellness, personal choice, and mind-body-spirit.
  • Provide support strategies and referrals for services to improve resident outcomes for chronic disease management and acute illness or injury.
  • Participate in the collaborative care weekly meeting and communicate with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.
  • Oversee the post-acute discharge planning and follow-up support process, collaborating with health center and hospital staff to coordinate admissions, transfers, and discharges.
  • Attend to medical emergency calls and partner with residents to prevent further incidents through risk reduction strategies.
  • Complete incident reports as per community protocol.
  • Provide annual updates for medical demographic information and advance directives.
  • Organize and facilitate vaccination clinics.
  • Encourage utilization and support provision of service with on-site healthcare practitioners.
  • Partner with Community Life Director and Resident Services Director to plan, implement, and evaluate Wellness Programming, health screenings, and health education opportunities.
  • Pro-actively build relationships with residents and seek to establish trust, respect, and connection.
  • Communicate effectively with families and staff, fostering and driving measures to improve community culture.
  • Maintain records appropriate for independent living and assure confidentiality of all resident information contained therein.
  • Attend in-service training and educational sessions as assigned.
  • Strive to achieve high levels of resident satisfaction, using sound judgement and strong organizational skills.
Requirements:
  • Active RN/LPN license for applicable state.
  • Three years of experience in a related organization preferred.
  • Excellent observation and communication skills, both written and verbal.
  • Self-directed with the ability to work with little supervision.
  • Flexible and cooperative in fulfilling obligations.
  • Ability to problem-solve, make ethical recommendations, and define appropriate boundaries.
  • Complies with accepted professional standards and practice.
  • Must be comfortable with using computers and have basic skills with computer platforms such as Word and Excel.
