Job Description
We are seeking a highly skilled Care Specialist - End-of-Life Support to join our team. In this role, you will have the opportunity to make a real difference in the lives of patients and families by providing emotional support and psychosocial evaluations during their end-of-life journey. Your expertise will be invaluable in helping us deliver the highest quality care and creating a positive experience for all those involved.
Responsibilities
* Provide emotional support and counseling to patients and families
* Conduct psychosocial evaluations to identify needs and develop personalized care plans
* Work closely with our interdisciplinary team to ensure comprehensive care delivery
* Participate in ongoing education and training to enhance skills and knowledge
Requirements
* MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
* 2+ years of clinical work experience, preferably in healthcare or hospice
* Strong communication and interpersonal skills
* Ability to sit, stand, bend, move intermittently and lift at least 25lbs effectively
Salary and Benefits
The estimated salary range for this position is $60,000-$80,000 per year, depending on experience. Our benefits package includes:
