Job Overview

Benchmark Senior Living seeks a dedicated Licensed Practical Nurse (LPN) to provide exceptional care to our residents in a warm and home-like environment.

About the Role

This part-time 2nd shift position is available in our Dementia Care Unit, requiring a compassionate and detail-oriented individual to deliver high-quality nursing care. As an LPN, you will be responsible for assessing, communicating, and documenting residents' status, as well as observing and reporting any significant changes in behavior and health.

Key Responsibilities
  • Assessment and Documentation: Accurately assess residents' needs and communicate effectively with the care team to ensure seamless delivery of care.
  • Observation and Reporting: Monitor residents' behavior and health, reporting any concerns to the Resident Care Director, physician, family, RCAs, and Executive Director as per state regulations.
  • Care Coordination: Direct and supervise daily work assignments of Resident Care Assistants, closely mentoring and guiding Lead Resident Care Assistants in all aspects of their job.
  • Case Management: Coordinate care needs with community providers through effective case management processes.
  • Supporting Independence: Balance service demands with supporting resident independence, recognizing and assisting in the prevention of elder abuse, neglect, and exploitation.
  • Emergency Response: Respond promptly to emergencies and personal emergency response systems, knowing when to call for backup support.
Requirements
  • A current state license as an LPN and CPR certification.
  • Graduate of an approved LPN program (per state requirements).
  • Minimum of 1-2 years of experience working as a nurse, preferably in a long-term care setting.
  • Prior experience working with the elderly in a paid or volunteer position is highly desirable.
Estimated Salary: $55,000 - $65,000 per yearBenefits
