**Job Summary:**

Phaxis is seeking a per diem Physical Therapist to join our team. As a per diem Physical Therapist, you will provide physical therapy services to patients in Augusta, Georgia.

**Responsibilities:**

* Provide physical therapy services to patients in a per diem capacity
* Work collaboratively with other healthcare professionals to develop and implement patient care plans
* Conduct initial assessments and evaluations to identify patient needs and goals
* Develop and implement individualized treatment plans to address patient needs and goals
* Provide education and training to patients and their families on physical therapy techniques and strategies

**Requirements:**

* 3+ years of experience as a Physical Therapist
* Ability to work in a per diem capacity
* Willingness to float between Augusta and Thompson
* Salary range: $80k-$100k
