Key Responsibilities- Provide evidence-based individual therapy services to patients, focusing on their identified needs and goals.
- Conduct comprehensive assessments, diagnoses, and treatment planning to ensure effective care.
- Develop and implement individualized treatment plans, incorporating brief therapeutic intervention models as needed.
- Provide crisis intervention services to patients, ensuring timely and effective support.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and discharge planning.
- Participate in clinical supervision, peer review, and performance improvement activities to enhance clinical practice and patient outcomes.
- Develop and maintain strong relationships with patients, families, and community stakeholders to promote education and outreach.
- Accurately document patient information, treatment plans, and outcomes in accordance with departmental procedures.
- Master's degree in Social Work, Psychology, or a related field.
- 1 year of experience in providing therapy services in a behavioral health setting.
- LCPC or LCSW licensure in the State of Illinois.
