Job Summary

We are seeking a highly skilled Geriatric Advanced Practice Provider to join our team at Catholic Health Initiatives. As a key member of our healthcare team, you will be responsible for providing high-quality patient care to our geriatric patients.

Responsibilities
  • Obtain medical histories and perform physical exams to assess patient needs.
  • Diagnose and treat chronic diseases, order diagnostic tests, and prescribe medications as needed.
  • Provide preventive health screenings and immunizations, and promote positive lifestyle habits through education and counseling.
  • Assist doctors with office surgeries, assure charts are signed off by the doctor within 7 days, and maintain patient confidentiality.
  • Make follow-up calls to check patients' progress, assist with phone coverage and hospital rounds, and work closely with other nurses to ensure efficient patient flow.
  • Consult with doctors regarding patient symptoms to ensure proper diagnosis and treatment plans.
Qualifications
  • Master's degree in nursing or Graduate of a Physician Assistant training Program.
  • 3-5 years clinical practice in an acute care setting required.
  • 1-3 years previous experience as an APP in an acute or primary care setting or equivalent experience.
  • Certified Nurse Practitioner, Certificate of fitness through American Academy of Nurse Practitioners, Licensed as an RN in the state of TN/GA, and Licensed as a CRNP in state of TN, or TN State PA License required.
  • BLS or ACLS required.
