This position emphasizes a collaborative approach to achieving shared objectives across various departments, thereby facilitating high-quality, patient-centered care.
Key ResponsibilitiesIn partnership with the Chief Nursing Officer, Director of Nursing, Director of Patient Engagement, and Chief Medical Officer, the Integrated Care Nurse will:
- Oversee the maintenance of comprehensive medical services, including the tracking, monitoring, and follow-up of referrals and patient communications.
Core Competencies:
- Proven experience in healthcare management.
- A minimum of three years in a leadership role.
- Exceptional written and verbal communication abilities.
- Strong critical thinking and problem-solving skills.
- Capacity to manage high-pressure situations and meet tight deadlines.
- Experience working with diverse, interdisciplinary teams.
Professional Experience and Education:
- Bachelor of Science in Nursing (BSN) is required; an advanced degree such as a Master of Science in Nursing (MSN) or Master of Public Health (MPH) is preferred.
- At least five years of nursing experience.
Licensure:
- Must possess a current nursing license in Connecticut.
