Job Summary

Curana Health is a leading provider of primary and post-acute care services for senior living communities. We are seeking a highly skilled and compassionate Senior Care Clinician to join our team.

About the Role

The successful candidate will be responsible for providing high-quality, patient-centered care to seniors in our assisted living, life plan communities, independent living, skilled nursing, and long-term care facilities. This will involve conducting clinical interventional and episodic direct care visits, evaluating and treating chronic and co-morbid illnesses, and coordinating care with collaborating physicians and other healthcare professionals.

Key Responsibilities

  • Conduct thorough assessments and develop individualized care plans for residents
  • Provide ongoing evaluation and treatment of chronic and co-morbid illnesses
  • Coordinate care with collaborating physicians and other healthcare professionals
  • Conduct Annual Wellness Visits and Annual History and Physicals as requested
  • Assist with facility-wide illnesses and coordinate care with other disciplines

Requirements

To be successful in this role, you will need to have a valid nurse practitioner or physician assistant license in the state of operations, current board certification in your field, and a Master's Degree in Nursing or Physician Assistant Studies. You will also need to possess strong leadership, interpersonal, and problem-solving skills, as well as excellent written and oral communication skills.
