Job Summary:
The Case Management Liaison supports the implementation of care management services for select patients in the community, working collaboratively with healthcare providers and clinical staff to ensure seamless coordination of care and follow-up.
Key Responsibilities:
- Assist patients and caregivers in navigating community resources to reduce barriers to care and improve health outcomes.
- Perform outreach and intake interviews to collect patient-level information, promoting optimal health and self-management through advocacy, empowerment, and identification of and connection with resources.
- Facilitate efficient service delivery and promote a positive patient-provider relationship.
- Assist with administrative support tasks.
Requirements:
- 2 Years of Clinical Experience in Acute Care or Healthcare Required.
- High School Diploma or Equivalent Required.
- Associate's Degree in a Related Field Preferred.
- Certified Health Coach Preferred.
Work Schedule:
Monday-Friday, with no weekend or call requirements.
