Habilitation Specialist

Job Summary:

The Habilitation Specialist will work closely with individuals with developmental disabilities to promote independence and self-advocacy. This role involves developing and implementing personalized support plans, coordinating community-based services, and providing ongoing guidance and encouragement.

Essential Responsibilities:

• Conducting regular assessments to identify individual needs and goals.
• Developing and implementing support plans tailored to each participant's requirements.
• Collaborating with healthcare providers and other stakeholders to ensure seamless service delivery.
• Facilitating communication and social interactions among participants and their families.
