About the Job

We are seeking a dedicated PCA care professional to provide direct service and case management services to individuals with developmental disabilities. This position provides skilled ongoing support to clients living in residential settings while accessing various community settings.

  • Provide direct client services to individuals, working collaboratively with Residential Coordinators, to ensure residential goals are obtained
  • Transporting and assisting clients in accessing various community settings, including grocery shopping, medical appointments, and courtroom settings
  • Complete daily tracking and necessary documentation to ensure compliance and company standards are met
  • Assist in the completion of assessments, pre-IP documents, medical documentation, financial documentation, and forms as needed
  • Routine home visits to assist clients in maintaining their apartments and overall wellbeing
  • Provide day program services and arrange weekly activities such as BINGO, craft projects, meal prep, and cooking, and community outings
  • Support family caregivers as needed
  • Implement individualized life skills curriculum
  • Maintain a positive attitude and calm demeanor during difficult situations, modeling effective crisis management and problem solving
  • Model appropriate skills and communication when working with clients with developmental disabilities and their teams
  • Ensure a positive quality of life and community integration for current clients
  • Maintain positive relationships with clients, guardians, and team members to ensure clients maintain their services
  • Advocate for client needs and services to ensure clients build skills and work toward residential goals
  • Evaluate client paperwork to ensure successful billing and claims testing
  • Communicate effectively with all team members and establish a comprehensive case management approach
