Job Title: Long Term Acute Care Registered Nurse

We are seeking a skilled Long Term Acute Care Registered Nurse to join our team at a top healthcare facility in Erie, Pennsylvania.

Job Summary:

The ideal candidate will have a strong background in Long Term Acute Care nursing, with experience in caring for patients with complex medical needs. The successful candidate will be able to work independently and as part of a team, providing high-quality patient care and collaborating with other healthcare professionals to achieve optimal patient outcomes.

Responsibilities:
  • Provide direct patient care to patients with Long Term Acute Care needs, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Monitor and manage patient medications, including IV administration and titration.
  • Perform routine medical procedures, such as wound care and dressing changes.
  • Document patient information accurately and thoroughly, including progress notes and medication administration records.
Requirements:
  • Active RN license in the state of Pennsylvania.
  • Minimum 18 months of experience in Long Term Acute Care nursing.
  • Experience with ventilators, tracheostomies, and other life-sustaining therapies.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
