Care Management Role
We are seeking a skilled Pediatric Social Worker to join our Care Management team. As a key member of our team, you will be responsible for planning and prioritizing care for individuals and populations of clients, focusing on strategies that promote optimal health within populations.
Key Responsibilities:
- Assess internal referrals and provide individual, short-term therapy services or care management services
- Develop plans of care and make recommendations to PCPs, specialists, and other members of the healthcare team
- Respond to patients' urgent mental health concerns and assist them in accessing appropriate levels of care

Qualifications
- 2 years of relevant experience in the field
- LICENSED - Master Social Worker (MSW) - State of Michigan
- Required Master's Degree in Social Work
