Renal Care Specialist

We are seeking a skilled Renal Care Specialist to join our team at Fresenius Medical Care. In this role, you will be responsible for providing high-quality psychosocial services to patients undergoing dialysis treatment.
About the Role- Assess patients' psychosocial status and identify areas of need that may impact rehabilitation and treatment outcomes.
- Develop and implement effective care plans in collaboration with patients and healthcare teams.
- Provide education and support to patients and their families on kidney disease and related topics.
- Collaborate with interdisciplinary teams to ensure comprehensive quality care and improve patient outcomes.
- Participate in process improvement activities to enhance patient satisfaction and quality of life.
- Masters in Social Work degree required.
- State-issued license to practice as a social worker.
- 2-5 years of experience in a related field.
- Excellent communication and interpersonal skills.
- Able to work independently and collaboratively as part of a team.
The salary for this position is estimated to be around $60,000-$80,000 per year, depending on location and experience. Additionally, we offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
