Job Overview

Human Touch Home Health Care is seeking a skilled Quality Assurance Specialist to ensure the quality and reliability of our home healthcare services. As a key member of our team, you will be responsible for designing and implementing tests, identifying defects, and collaborating with teams to resolve issues and deliver seamless experiences to our clients.

Key Responsibilities:

  • Develop and implement quality assurance processes to ensure compliance with regulatory requirements and industry standards.
  • Collaborate with clinical staff to provide training on CMS and state-specific guidance, as well as education on regulatory requirements and compliance.
  • Establish and implement home healthcare-focused quality and performance improvement efforts in accordance with systems used (EVV, EMR, etc.).
  • Train and supervise team members to ensure documentation of timely, fiscally sound, and medically necessary care for our clients and organization.
  • Partner with administrators and DONs to support continuous accreditation readiness, including mock surveys, inter-cycle monitoring, audits, and other activities.
  • Monitor and assist in preparing individualized clinical reports to help clinical management teams improve patient care and outcomes.
  • Support peer review efforts for nurses and caregivers on quality data, accreditation, and regulatory standards.
  • Support activities related to client safety incidents and findings.
  • Review past incidents, claims, and liability reports to identify risks facing the office.
  • Gather national and statewide data to provide comparison and guidance for developing new policies and procedures for improvement.
  • Provide recommendations and solutions to immediate quality assurance problems.
  • Create a risk management plan and conduct risk management training to clinical staff to help prevent future problems.
  • Develop new policies and procedures.

Requirements:

  • Current, unencumbered, active RN license in the state.
  • Home healthcare experience.
  • Current CPR and first aid certification.
  • 1 year of quality assurance experience.
  • Experience in OASIS.
  • Strong knowledge of clinical operations, CMS, and state regulations.
  • Management experience in the healthcare industry.
  • Experience interacting with multiple disciplinary teams.
  • Proficient in Microsoft Office Suite.
  • Excellent communication, relationship building, and interpersonal skills.
  • Excellent time management, organizational, and priority setting skills.
  • Excellent analytical and problem-solving skills.
  • Strong communication and collaboration skills.

Preferred Qualifications:

  • Certified Professional in Healthcare Quality Certification (CPHQ).
