Key Responsibilities:- Provide direct patient care to residents, administering medications and treatments as prescribed.
- Assist the Director of Assisted Living with 24-hour accountability for resident care.
- Perform admissions, including assessments and documentation on all residents.
- Participate in the care planning process and maintain the accuracy of care plans between scheduled reviews.
- Collaborate with Physicians, Pharmacists, Dietitians, Therapists, Activities, and Social Services to facilitate communication and execution of the total plan of care.
- Observe and promote infection control measures, as well as the facility's goals of Restraint-Free Environment and Limited Lift Environment.
- Protect patient confidentiality in all aspects of care and communication.
- Perform and document assessments of residents, including behavior monitoring, physical assessment, and response to specific medications.
- Assist physician in examination of resident or with treatments being performed by the physician.
- Respond to emergency calls to the Village or Willows if needed.
- Assist with principles and goals of Restorative Nursing by accountability in restorative procedures as indicated by the plan of care and performed by CNA's, PCA's, and Memory Care Assistants.
- Perform other duties as assigned by the Director of Assisted Living or Memory Care Coordinator.
- High School Diploma, GED or equivalent.
- Graduate in good standing of an accredited school of nursing.
- Current and unencumbered license to practice in the State of Illinois as a Registered Professional Nurse.
- Work experience in Gerontological Nursing is strongly desired.
- Medical/Surgical experience is acceptable.
- Working knowledge of healthcare delivery in the Geriatric, Rehabilitation, or Skilled Nursing setting.
- Knowledge of the aging process, pharmacology, assessment of the elderly, and practical application of clinical interventions.
- Current CPR certification.
