Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Theoria Medical, providing high-quality primary care services to patients residing in skilled nursing facilities (SNFs). As a key member of our post-acute care team, you will be responsible for rounding and managing the clinical care of our patients, prescribing medications and treatments, and providing comprehensive primary care services.

Responsibilities
  • Rounding daily to manage patient care and address acute/emergent issues
  • Prescribing medications and treatments for residents residing at SNFs
  • Providing high-quality, patient-centered care to patients residing in SNFs
  • Timely documentation of all patient care/encounters in Theoria's proprietary electronic health record (EHR) system, ChartEasyTM
  • Availability for phone/secure messaging consultations with facility nursing staff
Requirements
  • Current licensure as a Nurse Practitioner in the state of practice
  • Experience in primary care and post-acute settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Reliable transportation and timely attendance
