Case Manager

We are seeking a skilled Case Manager to join our team at UnitedHealth Group. As a Case Manager, you will play a critical role in providing Long-Term Services and Supports to our members, ensuring they receive the care and support they need to live healthy and independent lives.
Key Responsibilities- Conduct intake and screening to identify member needs and develop support plans
- Assess and reassess member needs, making recommendations for services and supports
- Develop and implement care plans, monitoring progress and making adjustments as needed
- Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
- Document all interactions and case management activities in our Department-prescribed system
- Bachelor's degree in a human behavioral science field, such as social work, psychology, or public health
- 5+ years of experience in Long-Term Services and Supports, or a combination of education and experience
- Excellent communication and interpersonal skills, with the ability to work with diverse populations
- Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments
- Access to reliable transportation and the ability to travel up to 75% within assigned territory
- Experience as a caseworker or case manager with a Long-Term Services and Supports population
- Knowledge of Medicaid and other government programs
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
