As a Registered Nurse in Home and Community Based Services, you will:
- Provide personalized care to patients in their homes, promoting independence and dignity.
- Collaborate with other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to ensure patients receive the best possible care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.

Responsibilities
As a Registered Nurse in Home and Community Based Services, your responsibilities will include:
- Providing high-quality clinical services within the scope of practice and in accordance with federal and state regulations.
- Completing thorough assessments and evaluations to ensure patients receive the best possible care.
- Developing and revising individualized care plans in collaboration with other healthcare professionals.
- Ensuring plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed.
Requirements
To be considered for this opportunity, you must have:
- A minimum of one year experience as an RN.
- Current RN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
