PRN Continuous Care LPN

Job Summary

Empath Health is seeking a skilled Licensed Practical Nurse (LPN) to provide continuous care to patients in various counties. The ideal candidate will have experience in an acute care setting and be able to work independently under the supervision of a Registered Nurse (RN).

Key Responsibilities
  • Provide bedside clinical observations and prescribed care to patients as directed by the RN case manager.
  • Process physician orders to maximize patient comfort and report significant changes in patient condition and family needs.
  • Collaborate with the RN to develop and implement patient care plans.
Requirements

To be successful in this role, applicants must be a graduate of an approved nursing program and hold a current Florida LPN license. A minimum of one year of experience in an acute care setting is required, with previous experience as a hospice LPN preferred.
