About the Role

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team as a Home-Based Medical Care Professional. As an Advanced Practice Clinician, you will play a critical role in providing high-quality, patient-centered care to our patients in the comfort of their own homes.

Key Responsibilities
  • Manage Chronic Illnesses: Provide comprehensive care to patients with complex, chronic conditions, focusing on wellness, prevention, and disease management.
  • Document Patient Information: Accurately and timely document patient information, including HEDIS quality gap measures and HCCs.
  • Engage New Patients: Conduct comprehensive geriatric-focused initial assessments, including functional evaluations, social and safety assessments, cognitive screening, and mental health assessments.
  • Triage and Diagnose: Triage, diagnose, and treat patients on an urgent basis in their homes, utilizing an array of equipment and medications.
  • Order and Interpret Tests: Order and interpret appropriate tests, including x-rays, labs, and EKGs, to diagnose and treat patients within your scope of practice.
  • Collaborate with PCPs and Specialists: Collaborate with patients' primary care physicians and specialists as needed to ensure comprehensive care.
Requirements
  • Graduate of an Accredited NP or PA Program: Must be a graduate of an accredited Nurse Practitioner or Physician Assistant program and hold certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA).
  • Active and Unrestricted State License: Must hold an active and unrestricted state Nurse Practitioner license or Physician Assistant (PA-C) in the state where the job is located.
  • Valid DEA License: Must have a valid DEA license or be able to obtain one prior to employment.
  • Reliable Transportation: Must have access to reliable transportation and comply with Optum's Motor Vehicle Safety policy.
Preferred Qualifications
  • 2+ Years of Clinical Experience: 2+ years of clinical experience in IM, Geriatrics, Primary Care, ED, Urgent Care, or similar settings.
  • Awareness of UM Standards: Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems.
