As a Home Care Assistant, you will be instrumental in delivering support to participants within their residences while collaborating closely with our Interdisciplinary Team (IDT) for comprehensive care planning. Key Responsibilities:

  • Deliver personal assistance and support with daily living activities, including bathing, grooming, dressing, and toileting
  • Assist participants with household tasks such as laundry, light cleaning, and grocery shopping
  • Monitor and document vital signs of participants
  • Prepare meals and assist with feeding, ensuring proper nutrition and hydration
  • Facilitate safe transfer methods and act as a medical escort when necessary
  • Implement home exercise routines as prescribed by the IDT
  • Provide translation services when needed
