About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Nurse Case Manager. As a key member of our care team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.

Responsibilities
  • Perform patient assessments and develop individualized plans of care in collaboration with the care team.
  • Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
  • Promote patient health and independence through teaching and rehabilitative measures, assisting patients in learning self-care techniques.
  • Supervise LPNs and HHAs, ensuring high-quality care is delivered.
  • Complete timely and accurate documentation, regularly communicating patient progress to the clinical manager and care team.
  • Plan and provide staff education, participating in clinical development and continuing education programs.
  • Perform on-call responsibilities and provide on-call services to patients/families as assigned.
Requirements
  • One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
