Job Summary

We are seeking an experienced Senior Registered Nurse to join our Home Health team. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for providing exceptional patient care.

Responsibilities

  • Assess patients' health needs and develop personalized care plans in collaboration with physicians and other healthcare professionals.
  • Provide direct patient care, including administering medications, wound care, and education on disease management and self-care.
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions between home and hospital settings.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Foster a culture of excellence by adhering to best practices, staying up-to-date on industry developments, and participating in ongoing professional development.
  • Mentor and educate new staff members, ensuring they receive comprehensive training and support.

Requirements

  • Current California Registered Nurse License.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Minimum 1 year of professional nursing experience, preferably in home health or a related field.
