Job Title: Hospice Case Manager

About the Role:

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. In this role, you will provide supportive care and services to patients and their families, working collaboratively with our interdisciplinary team to maximize comfort and health.

Key Responsibilities:

  1. Clinical Practice: Assess patient/family needs, develop individualized plans of care, and implement interventions to promote optimal health outcomes.
  2. Communication and Collaboration: Communicate effectively with attending physicians, other hospice staff members, and external agencies to coordinate care and utilize resources efficiently.
  3. Caseload Management: Maintain regular communication with the Administrator to review caseload, and collaborate with the attending physician to address patient/family care needs.
  4. Documentation and Record-Keeping: Maintain up-to-date patient records, accurately documenting problems, plans, actions, and goals.
  5. Ongoing Education and Support: Participate in ongoing education and training programs to enhance professional growth and stay current with best practices in hospice care.

Requirements:

  1. Licensure: Current licensure to practice as a Registered Professional Nurse in Florida.
  2. Experience: Minimum two years of recent varied work experience as a professional nurse, preferably one year in an acute setting.
  3. Skills and Qualifications: Demonstrated ability to assess and respond to patient/family needs, nursing skills, emotional intelligence, and tolerance for diverse lifestyles.

Work Environment:

This role requires travel by automobile and exposure to changing weather conditions. The employee works in various residential environments based on assigned caseload.

The estimated annual salary for this position is $80,000-$110,000, depending on experience and qualifications.
