Job Summary

The Triage Nurse Specialist is responsible for providing high-quality patient care and support to patients and community partners through telephone triage and advice services. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Respond to patient calls and provide accurate and timely advice and guidance on next steps
  • Collaborate with case management, nursing teams, and other departments to ensure quality patient care
  • Document calls and patient interactions in a timely and accurate manner
  • Develop and maintain relationships with community partners and healthcare providers
  • Stay up-to-date on relevant medical knowledge and best practices
Requirements
  • Bachelor of Nursing degree from an accredited nursing school
  • Minimum two (2) years of experience with direct patient care
  • Current State of California Registered Nurse License
  • CPR or BCLS certification
  • Experience in a community healthcare center or outpatient primary care setting
  • Strong verbal and written communication skills
  • Strong interpersonal and customer service skills
  • Strong computer and data entry skills
  • Strong analytical and problem-solving skills
